Child Find Notice 

It is the Page Unified School District’s responsibility to inform the general public and all parents within our boundaries of our responsibility to make available special education services for students with disabilities aged 3-21 years and how to access those services.  In addition, we have a responsibility to provide information regarding early intervention services for children birth through 2 years.  We are responsible for identifying, locating, and evaluating all children with disabilities, including children aged 3 through 21, and referring children from birth through 2 years of age to Arizona Early Intervention Program (AzEIP) for evaluation and appropriate services.  

We are also responsible for providing a free and appropriate public education (FAPE), which includes special education and related services to children with disabilities at public expense under public supervision and direction without charge to the parents. 

 

For all preschool and kindergarten students, and any student enrolling without records of screening, evaluation, and progress in school, the classroom teacher will complete screening activities within 45 days of enrollment.  

For a student transferring into a school, PUSD shall review enrollment data and educational performance in the prior school to determine if there is a history of special education for a student not currently eligible for special education or poor progress.  

If a concern about a student is identified through screening procedures or record review, the student's name shall be submitted to the administrator for consideration of the need for a referral, a full and individual evaluation, or other services.  The parents of the student shall be notified of the concern within ten (10) school days and informed of the District’s procedures to follow up on the student's needs.
